Private Pay Rates
$150- Diagnostic Assessment
$100 per 50-minute session
Nexus Letter: $300 (simple) *costs can increase
**Fees may increase depending on the complexity of the case, the number of records reviewed, the documentation requested, and the time required.**
Initial FMLA (straightforward): $150–$250
Short-Term Disability: $350–$600+
Form updates/revisions: $75–$150
Rush requests: additional $100–$200
** Sliding scale rates are offered to a limited number of clients who demonstrate financial need.**

Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your provider to verify how your plan compensates you for psychotherapy services. I’d recommend asking these questions to your insurance provider to help determine your benefits:

Does my health insurance plan include mental health benefits?

Do I have a deductible? If so, what is it and have I met it yet?

Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?

Do I need written approval from my primary care physician in order for services to be covered?
Payment
I accept all major credit cards and HSA as payment. Superbill can be provided for FSA reimbursement.
Administrative Services
FMLA and workplace accommodation paperwork are available for established clients when clinically appropriate. Clients must complete a minimum of 3 sessions prior to paperwork completion. Additional fees may apply, and completed forms may require 7–10 business days for processing.
Cancellation Policy
If you are unable to attend a session, please cancel at least 24 hours in advance. Otherwise, you will be charged a $75 missed appointment fee.
